Partnership with INIST-CNRS will create a new national archive of open
access research
BioMed Central and INIST <http://www.inist.fr/> (Institut de l'Information
Scientifique et Technique) are set to create a new archive of open access
research. Currently, all the research articles published in BioMed Central's
journals are archived, without delay, at the United States National Library
of Medicine's PubMed Central site <http://www.pubmedcentral.nih.gov>. The
INIST national archive will be another way in which the information
published in BioMed Central's journals is securely archived.

Europe

I recommend: K Ball, et al.: Effects of cognitive
training interventions with older adults: a randomized

Huh?!?!? you might be asking. Yes, bear with me:
This is a study to test whether memory training delays
the onset of dementia (and Alzheimer's). When
patients were tested afterward for how well they
memorized (call a 'proximal outcome'), the NNTs were
excellent, like 3 and 9. This is common sense,
practice makes perfect, the more you teach somehow
mnemonics, the better they memorize. (See the top
part of Table 1.)
However, when patients' activities of daily living
were tested (called a 'primary outcome'), the NNTs
were miserable, like 100, and the confidence intervals
wide. Again, indeed, this is common sense; the
subjects were only studied for two years, this is not
enough time to analyze dementia prevention. (See the
bottom part of Table 1.)
So, this article is an excellent example of why EBM is
important. The authors do not hide the preliminary
nature of these results, but they boast about the
unique strengths of this study (e.g. many patients,
RCT, diverse population). So, a cursory reading of
this paper may lead a busy resident to ask their
elderly patients to do more memorizing, pattern
recognition, and video games for speed. However, the
NNTs for performance outcomes ("primary outcomes")
make clear that such recommendations are premature.

remember that when I went though it initially I
thought that the way they got from the data to the
conclusions is a very interesting leap---maybe not
actually a "bad" article but give it a try yourself
and see what you think---just play the design and the
numbers.

I took EBM class from -- and JG was one of our
instructors. When we evaluated the following trial,
she send me the comment which I found very true. You
may want to look at this article.

This is an interesting study with some potential
flaws. Did you notice that the authors were also
consultants for Proctor & Gamble -- who manufactures
Olestra? Also I found the placebo effect to be very
powerful in this study -- 44% of the patients who
thought they were eating olestra chips, reported
gastrointestinal problems even though they were
actually eating regular chips! (While only 29% who ate
regular chips and assumed that they were regular chips
reported problems.) There was an accompanying
editorial that was very interesting. I have my doubts
about this study and would probably not use it.
